Program Description:
Provides medical care and social support for Government Assisted Refugees whose medical needs may be complicated by language, cultural, and knowledge barriers. The clinic focuses on providing health management, education and self-management support for those whose medical needs may be complicated by language, cultural, and knowledge barriers. Interpreters are available for appointments as needed, in over 150 languages. A referral is required from Bridge Clinic. This is not a walk-in clinic and service is by appointment only. The clinic is staffed by a full-time nurse practitioner who works collaboratively with physicians and community service providers. The clinic is co-located with Global Family Care Clinic.
English with interpreters available in person or by telephone.
